Dr. Rai is no longer accepting Medicare as of July 1, 2013

Dear Medicare Patient,

Thank you for sharing your health journey with me, and I look forward to continuing this journey in the future. However, as of July 1, 2013, I will be an opt-out Medicare provider. What this means is that you can continue to see me as a patient, but neither you nor I can bill Medicare for your visit. You will have to pay my usual fee of $220 for a 45-minute visit. Your lab work will continue to be billed as usual through Medicare, since Quest Laboratories is considered an “in-network lab”.

You will need to sign a contract on your next visit stating that you understand and agree to these terms and will not file the visit for reimbursement with Medicare.
